What the Tie Rod End Is
When you turn the steering wheel, the movement travels from the steering rack through the tie rod and finally into the tie rod end. That last link connects the linkage to the wheel: its internal ball joint keeps the steering angle flowing to the wheel accurately even as the suspension rises and falls. It is a small part, yet it is the connection that decides which way your car actually goes.
Why It Wears
The ball joint works under constant movement and is sealed by a protective rubber boot. Once that boot tears, water and grit get in, the grease is contaminated and the joint starts eating itself quickly. Rough roads, hard speed-bump hits and kerb contact all accelerate the process. A worn joint develops play, so your steering command reaches the wheel late and imprecisely.
